Informed Consent for Orthodontic Treatment
I understand and consent to the orthodontic treatment plan recommended by my orthodontist. Treatment will involve upper and lower fixed appliances (Phase II). The estimated treatment duration is 18 to 24 months. I acknowledge that actual treatment time may vary depending on individual response.
The total fee for treatment is as quoted on the treatment plan. An initial payment is due at banding, with the remaining balance to be paid in equal monthly instalments. A late cancellation fee of $50 applies for appointments cancelled with less than 24 hours notice. A 1.5% surcharge applies to credit card payments.
I agree to maintain good oral hygiene throughout treatment, including regular brushing and flossing. I understand that poor oral hygiene may result in decalcification, cavities, or gum disease. I will attend all scheduled appointments and wear elastics or other appliances as directed.
I have read and understood the above information. I have had the opportunity to ask questions and all my questions have been answered to my satisfaction. I voluntarily consent to the proposed orthodontic treatment.
